Loneliness would be a direct manifestation of separation, so I can see that definitely being spiritual pain. Heartbreak/loss cause seemingly physical pain without a real physical component, so maybe again this is spiritual pain (intense separation).

I think those who are very spiritual but beneath the umbrella of a strict religious dogma might experience other levels of spiritual pain, especially if they are indoctrinated at a young age.
